Thank you for choosing Infectious Disease Associates. We look forward to meeting you. To help you prepare for your upcoming visit, please acquaint yourself with the following office policies, and download our forms to fill out and bring to your first visit. Thanks!

Your First Appointment

Our front office staff will answer your call and schedule your appointment after getting some preliminary information on the purpose for your visit, your medical insurance, referring physician, and your appointment preferences.

To streamline your visit, please download the new-patient intake forms from this Website and bring the completed forms with you. It is also important that you bring a list of the medications you are presently taking. Please plan to arrive 15 minutes early for your scheduled appointment.

Your first appointment will be scheduled with one of our Infectious Disease Specialists and will usually last one (1) to one and one-half (11/2) hours. During the appointment, our specialist will gather detailed information regarding your medical history as well as address your questions and concerns. Our specialist will also perform a physical examination and may prescribe some diagnostic testing in order to design your treatment plan.

Your First Appointment

Since we attempt to schedule patient appointments as quickly as possible, it is very important that patients keep their appointments or give us at least 24 hours’ notice of their need to either cancel or reschedule. Otherwise, another patient may be deprived of an opportunity to schedule an appointment. Our fee for missed appointments with insufficient notice is $60.00.
